UF Health social workers invite GOC colleagues to meet directly with patients who may benefit from the opportunities and community offered through the Clubhouse Model. These conversations can introduce something especially important during a difficult time: hope for a better future.

The care doesn’t end when someone leaves the hospital. UF Health wants its patients to have support and opportunities waiting for them in the community.
